Member forms:

Application Forms to Work with Children and accompanying Child Protection Policy for Members. If you are applying for a paid position with children you may also need the Minister Reference Letter Authorization Request. Background and reference checks must be satisfactorily completed prior to working with children. A copy of your driver's license and auto insurance are also required.
